Applications for membership should be submitted on the application form. The form now comes in two formats: One form is in PDF format and requires a PDF reader. PDF readers are available at no cost for virtually all platforms. A list is available here.
If you do not have a PDF reader, you can also view the application as a web page. Once you have loaded the form, please print it and answer the questions.
Dues are payable on a calendar year basis, up to two years in advance at the current rate, in either US or Canadian dollars. Rates for 2008 are $20 (US) or $21 (Canadian).
